Why it happens
- A large TIFF pyramid was opened in full
- A page file is capped
- A 32-bit reader was used
Steps that usually fix it
- Open the file in tiles
- Use a 64-bit reader
- Raise or uncap the page file

Use 64-bit readers for pyramids and large scans.
